甘油三酯高密度脂蛋白胆固醇酯比值和影像总负荷与脑小血管病认知障碍的相关研究

摘要

目的:探讨血液中甘油三酯高密度脂蛋白胆固醇脂比值(TG/HDL-C)和影像总负荷与非糖尿病的脑小血管病(Cerebralsmallvesseldisease,CSVD)认知功能的相关性。方法:回顾性分析2022年9月至2024年9月承德医学院附属医院全科医疗科住院治疗的279例CSVD患者临床资料。采用简易精神状态量表(MMSE)和蒙特利尔认知评估量表(MoCA)评估认知功能,根据结果分成认知正常组(129例)和认知障碍组(150例)。结果:CSVD认知功能障碍组患者血TG/HDL-C比值水平明显高于认知功能正常组(P<0.001)。血TG/HDL-C比值水平与CSVD影像总负荷呈正相关性,与认知功能呈负相关性。多因素Logistic回归分析表明,血TG/HDL-C比值是CSVD认知功能障碍的独立危险因素。ROC曲线示TG/HDL-C联合影像总负荷较单一标志物诊断价值更高。结论:TG/HDL-C与CSVD影像总负荷和认知功能均具有相关性,相较于单一生物学标志物联合影像总负荷对于早期识别CSVD认知功能的价值更突出。
