格隆溴铵对全麻下俯卧位脊柱手术患者肺功能影响的研究进展

摘要

随着中国人口老龄化的持续加剧，需要进行手术治疗的年长者数量也在逐步上升。这些患者中，全麻插管下接受手术的比例更高。特别是由于生活方式、坐姿等多种因素的影响，腰椎间盘突出手术成为主流手段。俯卧位能为腰椎间盘突出手术提供更好的视野，方便术者的操作，是腰椎间盘突出手术的一种常用体位。但术中俯卧位属于非生理性的体位，这种特殊体位对患者的呼吸功能有显著的影响。由于年龄的增长，老年人肺的顺应性逐渐降低，肺泡和气道弹性回缩力进行性下降，肺气肿的发生概率增加，肺泡过度膨胀，将进一步减少肺泡表面活性物质，可直接导致肺功能障碍。另外，随着年龄的增长，老年患者的胸廓适应性逐渐减少。这种影响不但会影响其顺应能力，还可以影响纵隔和横膈的弯曲程度，对呼吸动力学形成负面效果，导致需要进行胸廓移动所需的通气功也相应提高。研究发现格隆溴铵能让支气管平滑肌松弛，扩张气道，能够管控炎症因子的释放，进而调节体内炎症反应，增强肺部的顺应性，提升病人的肺部通气、肺部顺应性以及肺部功能，减少并发症的发生。
