血浆D-二聚体及凝血功能四项在辅助生殖技术妊娠后的检验价值

摘要

随着辅助生殖技术（ART）的大量应用，有关血栓并发症的报道逐渐增加，文献报道ART妊娠后并发血栓形成的发生率约为0.04%。越来越多的研究表明，促排卵药物的使用会凝血因子Ⅷ、组织因子、血管性血友病因子（VWF）活性增加，激活外源性凝血途径；抗凝血酶原Ⅲ、蛋白S、C等抑制因子合成与活性下降，减弱抗凝机制；纤溶酶原激活物抑制物（PAI-1）和组织型纤溶酶原激活物（t-PA）减少，纤溶活性增加。以上变化可表现为血栓前状态，临床表现为凝血酶原时间（PT）、活化部分凝血酶原时间（APTT）缩短，纤维蛋白原（FIB）、D-二聚体（D-D）活性水平升高。通过动态监测ART妊娠后孕早、中、后期的凝血功能变化，早期预警，提出治疗建议。
